If you think you might have dyslexia, dyspraxia, ADD, ADHD or an autism spectrum condition including Asperger Syndrome, or maybe your Academic Adviser, Tutor or friend has suggested this, please use the resources on this site to check it out.

1: Read the instructions before going any further.

2: Print the finance agreement, sign and hand in with your completed checklist(s) at the Student Support Centre reception or sign, scan and e-mail to disability@uea.ac.uk.

3: Download and handwrite the free writing form. This is your opportunity to tell us why you think you might have a specific learning difficulty or autism spectrum condition. hand in with your completed checklist(s) at the Student Support Centre reception or sign, scan and e-mail to disability@uea.ac.uk.

4: complete the pre-screening triage form. This will indicate which of the checklists below you should use.

Finally: download and complete the checklist(s) indicated by your triage responses and follow the instructions at the end of the form. You can complete more than one. The checklists should take 5-10 minutes each to complete.

If you have queries or concerns after completing the questionnaires and wish to discuss these with a Disability Adviser, please call 01603 592761 or drop in to the Student Services Centre reception to make an appointment.

You can return your checklists, finance form and free writing form by e-mail to disability@uea.ac.uk or in hard copy to the Student Services Centre reception.

How we use your information

By submitting this form you will be sharing some of your personal data with the University of East Anglia, and we are required by law to look after your information, and help you understand how it will be used.
